Who is Eligible

DVA home assessments are available to veterans and war widows/widowers who hold a Gold Card or a White Card (for specific conditions). If you find daily tasks becoming difficult, an OT assessment can help identify needs for equipment or minor home modifications.

Eligibility & Process

Referral Process

The process starts with a referral from your GP using DVA Form D904. Once they recommend an Occupational Therapy assessment, you or your family can contact us to schedule a convenient time for a home visit in the South East Queensland area.

Assessment Day

Nick will visit your home to observe how you move and manage daily activities. We discuss any challenges you face and recommend practical solutions, which may include grab rails, mobility aids, or strategies to increase independence and safety.
