Learn About the Assessment Process for drivers
We understand that driving assessments can feel confusing and daunting. Below you will find a step-by-step outline of what you need to do prior to your assessment and the assessment process from start to finish.
Please follow these steps to prepare for your appointment.

Contact Nick directly via call or email to discuss your assessment requirements.
Have your doctor or specialist complete the referral form and the QLD Transport Medical Certificate. Referral Form | QLD Transport Form
Have your Optometrist complete the required Optometrist clinical form for driving capability. Optometry Form
Submit your completed medical certificate and documentation to QLD Transport for processing.

Pre-Assessment Forms:
1. Doctor to complete QLD Medical Certificate (F3712) for Driving and Referral Form

2. Driver takes QLD Medical Certificate for Driving to Queensland Transport (TMR)
Â
3. Driver to see Optometrist to get the optometrist form completed

4. All forms and a copy of the Driver's license to be emailed to nick@seqot.com.au

Off-road Assessment:
6. Off-road assessment to be completed at the driver's home to assess physical, cognitive, perceptual and visual abilities to identify any factors that may impact safe driving.

On-road Assessment:Â
7. If safe to proceed, an-road assessment is conducted with a qualified driving instructor in a dual-controlled vehicle.Â

Post-Assessment:Â
8. OT to complete report with recommendations and send to the referring Doctor and Queensland Transport (TMR). Â
